Many therapist marketing agencies specialize in one channel: Google Ads. They are often very good at it. Paid search reaches people at the moment they type "anxiety therapist near me", and a well-built campaign can produce inquiries within days.

The question is whether ads alone are enough for your practice, or whether you need a broader system that also covers search visibility, your website, and intake. This guide lays out when each makes sense. If you want both handled together, our therapist marketing agency page explains how we combine them.

What an ads-only agency does well

A focused Google Ads agency typically handles:

    • Keyword research built around real client language.
    • Negative keyword lists that filter out job seekers, students, and unrelated searches.
    • Location targeting matched to where you are licensed.
    • Ad copy and landing pages for specific services.
    • Conversion tracking and monthly optimization.

When the rest of the practice is already working, this is efficient. You are buying demand and someone skilled is managing the auction.

    Where ads-only setups leak

Ads put a prospective client on a page. What happens next depends on things an ads agency often does not control.

    The website does not convert

If the landing page does not show who the client would see, what it costs, whether insurance covers it, and how to book, the click is wasted. Good ads agencies build dedicated landing pages, but the main site still matters because many clients browse before contacting you. See therapy landing pages that convert.

    Intake is slow

Therapy inquiries go cold quickly. A client who emails three practices usually books with the first one that replies warmly. If your reply takes a day, a good campaign looks like a bad one. This is the most common leak we see, and it is invisible in an ads report. See convert therapy inquiries into clients.

    Nothing compounds

Ads stop when the budget stops. If every client comes from paid search, your cost per client never falls and you are exposed to rising click prices. SEO and a strong Google Business Profile build visibility that keeps working. See SEO for therapists.

    The math that decides it

A simple way to compare: work out your cost per new client, not cost per click.

    StepExample
    Monthly ad spend$1,500
    Cost per inquiry$50 (30 inquiries)
    Inquiry-to-booked rate35% (about 10 consults)
    Consult-to-client rate60% (6 clients)
    Cost per new clientabout $250

Now change only the intake step. If the inquiry-to-booked rate rises from 35% to 65%, the same spend yields about 11 or 12 clients, and cost per client falls to roughly $130. No change to the ads at all.

That is why a full system often outperforms better ad management alone. Use our Google Ads calculator for therapists to run your own numbers, and see Google Ads cost for therapists in Ontario for local benchmarks.

When ads-only is the right choice

    • Your website already converts well and shows fees, clinicians, and booking clearly.
    • You or your admin reply to every inquiry within a few hours.
    • You already rank in the map pack and want extra volume.
    • You need to fill a new associate's caseload quickly and SEO can follow later.

When you need the full system

    • You are getting clicks or inquiries but not enough booked consults.
    • You rely entirely on Psychology Today or referrals and have little Google visibility.
    • Your website is more than a few years old or was built without a booking path.
    • You are a group practice and need demand routed to specific clinicians by concern and availability.

    How to evaluate either option

Ask any agency, ads-only or full-service:

    1. Do you track booked consults, or only form fills and calls?
    2. Will you look at our intake response time and process?
    3. Who owns the Google Ads account?
    4. What is the management fee, separate from ad spend?
    5. What would you change on our website before launching ads?

An ads specialist that asks about your intake is a good sign. A full-service agency that cannot explain your cost per client is not.

    Bottom line

Google Ads is often the fastest lever for a therapy practice, and a specialist ads agency can be a great hire when everything after the click already works. When it does not, the bigger gains usually come from the website, intake, and search visibility around the ads. Fix the system first, then buy traffic into it.
